Erythematous Eruption of the Lower Abdomen in a Young Athlete

Diagnosis: Allergic contact dermatitis to textile dyes and rubber accelerators in workout clothing (textile contact dermatitis)

A 29-year-old woman presents with a several-week history of an itchy, well-demarcated, blanching erythematous eruption confined to the lower abdomen and bilateral flanks corresponding precisely to the waistband and side seams of her dark synthetic workout pants. The distribution, sparing of skin not in contact with the garment, and exacerbation with sweat and exercise point to allergic contact dermatitis to components of athletic clothing.

Clinical Presentation

A 29-year-old otherwise healthy woman presents to a community dermatology clinic with a 4-week history of a pruritic eruption on the lower abdomen and flanks. The rash is most intense after CrossFit workouts and showers and gradually fades over 24-48 hours when she wears looser cotton clothing. On examination there is a confluent, ill-defined erythematous to slightly papular patch involving the infraumbilical and suprapubic skin, fading in a curved geometric border that mirrors the upper edge of her dark spandex/polyester athletic leggings. Faint linear erythema is also present along the lateral flanks corresponding to the side seams of the garment. There are no vesicles, no scale, no excoriation crusts, and no involvement of the back, anterior thighs, or skin under the bra band. The umbilicus and skin under cotton underwear are spared. Distribution: Geometric, conforming exactly to the waistband contact zone of the leggings, with classic sparing of areas covered by cotton undergarments. Morphology: Blanching erythema with fine follicular accentuation; no vesiculation at presentation. Symptoms: Pruritus, occasional burning; worse with sweating and friction.

Clinical History

The patient began using a new pair of black moisture-wicking athletic leggings approximately 6 weeks before presentation. She trains 5-6 days per week, often in the same garment for 60-90 minutes of high-intensity exercise, and has not been changing immediately after workouts. No prior personal history of atopic dermatitis, asthma, or allergic rhinitis. No new soaps, laundry detergents, fabric softeners, body lotions, or sunscreens. No new metal jewelry or belt buckles in the affected area. Symptoms remit within 2-3 days when she wears loose cotton clothing and recur within 24 hours of returning to the leggings. No systemic symptoms: no fever, arthralgia, mucosal lesions, or generalized eruption. Family history is non-contributory. Medications include only an oral contraceptive.

Treatment

Acute Management Strict avoidance of the suspected garment and any other dark-colored synthetic athletic wear pending evaluation. Mid-potency topical corticosteroid (e.g., triamcinolone 0.1% cream) applied twice daily to affected skin for 7-14 days. Cool compresses and a fragrance-free emollient (petrolatum-based) for symptomatic relief. Oral cetirizine 10 mg nightly for pruritus. Counsel on showering and changing immediately after workouts to reduce sweat-mediated allergen leaching. Definitive Workup Refer for comprehensive patch testing including the North American Contact Dermatitis Group (NACDG) Standard Series, the textile dye and finish series (e.g., Disperse Blue 106, Disperse Blue 124, Disperse Orange 1/3, Disperse Red 1/17, Disperse Yellow 3, formaldehyde resins such as ethyleneurea melamine formaldehyde and dimethylol dihydroxyethyleneurea), and the rubber accelerator series (carba mix, thiuram mix, mercapto mix, mixed dialkyl thioureas) to evaluate for elastane/spandex-related allergens. Bring the suspect garment to the patch test visit; consider testing a piece of the fabric "as-is" using a wet textile patch test if standard panels are negative. Long-Term Strategy Once specific allergens are identified, provide a written allergen avoidance handout (American Contact Dermatitis Society Contact Allergen Management Program — CAMP — database is the standard tool). Recommend 100% cotton, undyed or naturally dyed athletic wear; pre-washing new clothing 2-3 times before wear; avoiding "wrinkle-resistant," "stain-resistant," or "anti-microbial" finishes which often contain formaldehyde resins. For athletes who require performance fabrics, consider merino wool or polyester garments certified to OEKO-TEX Standard 100, which restricts disperse dyes and formaldehyde.

Differential Diagnosis

Irritant contact dermatitis (friction/sweat): Common in athletes, but typically lacks the sharply geometric pattern that mirrors the garment edge so precisely and tends to involve high-friction zones (inner thighs, axillae) more than the waistband. Miliaria rubra (heat rash): Discrete pinpoint papulovesicles in occluded sweaty skin; lacks the sharply demarcated erythematous patch and resolves rapidly with cooling. Tinea corporis or tinea cruris: Usually has an annular configuration with a raised scaly active border and central clearing; KOH preparation is diagnostic. Nickel allergic contact dermatitis (belt buckle, button, or stud): Classic localized periumbilical erythema; ruled out here by a drawstring (no metal hardware) and by distribution extending to the flanks. Atopic dermatitis flare: Typically symmetrical in flexural sites with personal/family atopy; not garment-shaped. Inverse psoriasis: Well-demarcated red plaques in skin folds; typically chronic, not garment-conforming. Photocontact or photoallergic dermatitis: Distribution follows sun exposure rather than fabric edges. Cholinergic urticaria: Punctate wheals with exercise and heat; transient minutes-to-hour wheals, not a fixed patch.

Key Learnings

High-Yield Pearls Pattern recognition is diagnostic. A geometric eruption that precisely conforms to the contour of a garment is contact dermatitis until proven otherwise. Photograph the patient with and without the suspect garment. Disperse dyes are the #1 textile allergen. Disperse Blue 106 and Disperse Blue 124 are the most common culprits in dark blue, black, brown, purple, and green synthetic clothing. They are poorly bound to polyester and acetate fibers and leach onto sweaty skin during exercise. Formaldehyde-releasing textile resins (ethyleneurea melamine formaldehyde, dimethylol dihydroxyethyleneurea) are used for "wrinkle-free" and "easy-care" finishes and cause widespread eruptions in formaldehyde-allergic patients. Rubber accelerators in elastane (spandex) — carbamates, thiurams, and mixed dialkyl thioureas — should be tested when the eruption localizes to elastic waistbands, bra bands, or compression garments. Sweat amplifies the reaction by leaching disperse dyes and unreacted formaldehyde from the fabric, which is why athletes and people in hot climates are over-represented. Cotton is not always safe. Cotton blends are still treated with formaldehyde resins; "100% cotton, natural color" is the safest bet for proven textile-allergic patients. Patch testing must include a textile dye and finish series. The NACDG standard series alone misses up to half of textile contact allergies; always add the dedicated series and consider testing the actual fabric piece. If the rash looks like the clothes, it usually is the clothes.
