Best approach for refractory eczema in pediatric patients?

I have a 6-year-old patient with severe atopic dermatitis who has failed topical corticosteroids and calcineurin inhibitors. Parents are hesitant about systemic therapy. What approaches have worked for your difficult cases? Particularly interested in hearing about dupilumab experiences in this age group.
