Differential Diagnosis

• Squamous cell carcinoma in situ (Bowen disease) • Invasive squamous cell carcinoma • Seborrheic keratosis • Psoriasis (plaque) • Superficial basal cell carcinoma • Lentigo maligna

Key Learnings

• Considered a precursor to SCC with ~10% lifetime risk of progression per lesion • Field cancerization concept — treat the field, not just individual lesions • Tender/indurated AKs or those not responding to treatment require biopsy to rule out SCC • UV protection is critical for prevention and reducing recurrence • Dermoscopy: strawberry pattern (pseudonetwork with white-yellow scale)
